Costs

A prepayment of $799 will be collected prior to your lab and fertility assessment visits. In addition you will be billed for any co-pay, co-insurance, or co-payment as well.

If a semen analysis (SA) is done, an additional $111 is collected at the time of the SA visit.

Pre-conception counseling, assessment and screening may be considered elective by your insurance carrier and may not be covered. The costs to you will vary according to your insurance plan.

We accept and bill commercial plans. Please familiarize yourself with your specific coverage to understand your payment obligation and be prepared to provide your current insurance information at the time of your appointments.
