Mill City Clinic outcomes earn recognition

Mill City Clinic was rated as the top location in Minneapolis for diabetes care (based on data from Minnesota Community Measurement) in Consumer Report's October 2012 edition. In a special edition of its magazine, Consumer Reports published ratings of 552 Minnesota physician group practices and an analysis of Minneapolis-St. Paul medical groups focusing on cost and quality of care. The ratings were compiled in collaboration with the Robert Wood Johnson Foundation and Minnesota Community Measurement (MNCM), a nonprofit organization that has worked since 2004 to improve health care and cut unnecessary cost.

Mill City Clinic was also recognized by the Bridges to Excellence program. This clinic is one of five University of Minnesota Physicians clinics that have earned recognition and financial rewards from the Buyers Health Care Action Group through its Minnesota Bridges to Excellence program. This nationally recognized program rewards providers who achieve optimal outcomes for patients with diabetes, vascular disease and depression.

The ratings are based on quantifiable measures collected by the practices themselves on how well their patients control two of the most common and deadliest chronic diseases: diabetes and cardiovascular disease. Scores are based on the percentage of patients who achieve all of the targets for the best management of each of the two conditions. That means controlling both blood pressure and cholesterol levels and, in the case of diabetes, blood sugar levels, too. It also means not smoking and taking a low-dose aspirin to prevent heart attacks when appropriate.

Mill City Clinic is very proud of the feedback they get from patients. Through their patient satisfaction surveys, they consistently score high marks in patient satisfaction, receiving top scores from patients on quality of staff, overall rating of physicians and physician communication. More than 95 percent of patients would recommend this clinic to their friends and family.

Health Care Home
Mill City Clinic is a certified health care home, known nationally as medical home, and part of Minnesota’s 2008 health reform legislation. The goal of health care homes is to transform primary care; patients and families are at the center of their care, and the right care is provided at the right time, in the right place. It is a team approach that gives patients access to healthcare services and the support they need.

Our Team

Complex health care topic exploration

Hippocrates Cafe

Hippocrates Cafe productions use professional actors and musicians to explore complex health care topics, incorporating equal measures of humor and reflection.

Actors and actresses read from a variety of sources, including poems, short stories, novels, and scientific journals, while musicians perform instrumental interludes and other selections chosen to illuminate a specific topic. Creator and director Jon Hallberg, MD, hosts each show, providing context for performances.

To-date, Hippocrates Cafe has performed more than 15 shows in a variety of venues to audiences of 10-700. Topics have included cancer survivorship, care for the underserved, cold and flu, and eating disorders. Shows are non-prescriptive in nature and designed for radio play.
