Clinic Overview

The team at University of Minnesota Physicians Smiley’s Family Medicine Clinic provides a full range of services for your entire family.  The specialty of family medicine treats people of all ages, from newborns to seniors. Family medicine also includes care of patients through pregnancy and childbirth and the treatment of women’s health issues. At Smiley's, we welcome all patients, regardless of race, ethnicity or sexual orientation.

Along with our physicians, we use a team of educators, pharmacists and psychologists to teach patients how to prevent and manage disease. When the need arises for specialty care, we help patients through the referral process and continue to manage their care as needed. We are a Certified Health Care Home and offer enhanced care coordination services.

Our Team

Patient Information

For a life-threatening emergency, call 911 or go to the nearest emergency room. Contact your clinic doctor as soon as possible. The emergency room (ER) is at University of Minnesota Medical Center on Riverside Avenue. Please contact the clinic before going to the ER for emergencies that are not life threatening.

Call poison control: 612-347-7661 or 1-800-763-7661. Do not call the clinic.

After-hours care
You can reach one of our doctors 24 hours a day by calling the clinic. Please call 612-333-0770 and stay on the line for service.

Nurse line
A nurse can be reached at our main number (select option 2). Please be ready to give information such as:

  • Patient name
  • Date of birth
  • Temperature
  • Length of illness
  • Symptoms

A nurse will return your call as soon as possible, usually within two to three hours. If you need to speak to a nurse right away, please tell the person taking your call or press zero.

When scheduling an appointment, please tell us:

  • Which provider you’d like to see
  • Your symptoms
  • Insurance Information

If you need to come in for lab only, please schedule an appointment. Walk-in visits are available Monday through Friday, 8:30 – 10 a.m. and 1 – 2:30 p.m. If you need to cancel an appointment, please notify us as soon as possible.

Test results
If your doctor orders lab tests, you will either be called with the results (if not available at your appointment), or a letter will be sent with your results. If you do not receive your test results after two weeks, please call the clinic and ask to speak with a doctor or nurse.

Prescription refills
Please have your prescriptions refilled at your office visits. If you require refills before your visit:

  • Call your pharmacist
  • Allow 24-48 hours for refills

If the pharmacist cannot refill your prescription, please have your pharmacy contact the clinic during regular business hours. If you would like to pick up your prescription at a new pharmacy, please contact your new pharmacy to get the refill transferred there.

Interpreters are available at our clinic in a variety of languages. Please make your request at the time of scheduling.

Financial policy 
We accept most insurance plans. Check with your insurance provider to determine whether your care at Smiley's Clinic will be covered. If you have insurance: 

  • Bring your insurance card with you to each visit. 
  • Copayments are due at the time of service.

If you do not have insurance:

  • All patients are REQUIRED to make a payment on day of service.
  • All accounts with a balance due will receive a monthly statement.
  • A 20% discount will be given for every bill paid in full on the day of the visit.

Medical records
A valid release-of-information form is required by our Medical Records Department before records can be released. Please allow 10 days if possible.

If specialty care is required, your family doctor will:

  • Refer you to the appropriate specialist
  • Provide the specialist with important information concerning your care

A written referral will be completed for you by the patient representative. This is very important if you have insurance from UCare Minnesota, BlueCross/Blue Shield, Preferred One or Medica Premier.

Special services
Smiley's Family Medicine Clinic offers social service assistance and patient education. Your provider can also help arrange psychiatric care.
