Scheduling Office
Our scheduling office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. To help us schedule the right amount of time for your visit, please let us know:

  • which provider you want to see
  • why you are being seen
If you need to be seen on a same day basis, please:
  • call the clinic first
  • tell us why you need to be seen
This will allow our clinic staff to make the best arrangements for you. If you need to cancel an office visit, please notify us as soon as possible.

Please note: If you need to come in for lab only, please call the clinic for an appointment. This will allow our staff to pull your chart and will reduce the length of your visit time.

Telephone Calls
To help your provider understand your problem, please be ready to give information such as:
  • temperature
  • length of illness
  • symptoms
A nurse will return your call as soon as possible, usually within two to three hours. If you need to speak to a nurse right away, please tell the person taking your call.

Test Results
If your test results are not available on the same day the test is done and you do not have a follow-up office visit scheduled, please allow two weeks for the physician to contact you with your results.

Prescription Refills
Please have your prescriptions refilled at your office visits, or
  • call your pharmacist
  • allow 48-72 hours for refills
If the pharmacist cannot refill your prescription, call the clinic during regular business hours.

Referrals
If other specialty care is required, your primary care provider will:
  • refer you to a specialist, if it is medically appropriate
  • provide the specialist with important information concerning your care
A referral form, either written or online, will be completed for you by our patient representative. This is very important if you have insurance from UCare Minnesota or Blue Plus.

Medical Records
We strive to provide accurate and confidential medical records. If you need to transfer your records, a valid release of information form is required by our Medical Records Department before records can be copied. Please allow one week for copying, if possible.
