Community Care Program

Patients experiencing a financial hardship in paying for the cost of their medically necessary care may qualify for assistance under our Community Care program.

This program provides a discount for UMPhysicians charges. Care that is considered elective or cosmetic is not eligible for this program. Services billed by the facility, hospital, anesthesia providers, Dental Clinic or CSH Clinic are not eligible for this program.

Program Guidelines

Patients whose Annual Gross Income is at or below 275% of Federal Poverty Guidelines and assets do not exceed $15,000 (household of one) or $25,000 (household of two or more) may qualify for the program. Retirement funds will be included as assets.

Families with more than 8 persons, add $4,160 for each additional person.
