CoPayments

Your co-pay is due at the time of check-in.  We accept cash, personal checks, Visa, MasterCard, American Express and Discover credit cards.

PAYMENT OPTIONS

You may pay your bill online, over the phone or by mail. For your convenience we also offer the ability for you to receive your monthly statement online. To sign up for e-statements or to pay your bill online go to: UMP bill pay

Payments will be applied to the oldest outstanding balance at the time your payment is received.

For check payments, you authorize University of Minnesota Physicians and University of Minnesota Health Clinics and Surgery Center to use information from your check to make a one-time electronic funds transfer from your account, or to process a payment as a check transaction.

insurance

Our physicians participate in most major insurance plans and we will submit a claim on your behalf.  Please contact your health insurance carrier before your visit to understand coverage, benefits, out of pocket costs and if your appointment requires a referral or prior authorization.  

Insurance types accepted:

  • Contracted insurance plans
  • Medicare
  • Medicaid
  • Tricare/Champus
  • HMOs and PPOs
  • Other insurance such as motor vehicle and worker’s compensation.

Please check with your insurance to see if the clinic and doctor you are visiting participates with your insurance and if your plan is considered in-network.
We will submit charges for your appointment(s) to your insurance carrier. Once your insurance has processed your claims, if you have a balance you will receive a statement. Based on services and where you were seen, there may be instances when you receive more than one bill.
Your first statement will list detailed charges and following monthly statements will summarize any balance as a previous balance due. We recommend that you keep a copy of your initial statement for your records.

Preventative Visits and Annual Wellness Exams

Preventative care (or well services) promotes a healthy lifestyle to prevent health problems in the early stages.   The exam is prevention focused, not problem focused.

  • If you have a new health problem or other diagnoses that needs to be addressed during your preventative exam, the portion of your visit related to the treatment of your diagnosis would apply towards your deductible and co-insurance medical benefits.
  • If your provider feels that the majority of the visit was spent caring for medical concerns, the entire visit may be considered a medical treatment visit and would not be billed as preventative.  You are responsible for knowing what services are covered under your health plan.

Your provider does not know your health plan benefits and sees many patients with various insurance plans throughout the day.  For questions regarding your insurance, review your Summary of Benefits prior to your preventative exam or Call Customer Service for your benefit information.


Routine preventative care may include:

  • Past Medical, Social and Family History
  • Complete physical exam and review of body systems
  • Review of medications
  • Immunizations
  • Counseling/anticipatory guidance/risk factor reduction interventions
  • Review of age/gender appropriate screening tests

Important information regarding out-of-pocket Costs

Within our University of Minnesota Health system, we have both freestanding and hospital-based clinics.  For hospital-based clinics, most insurances process claims under the insured’s outpatient hospital benefits.  Depending on your health insurance benefits, this may result in increased out of pocket costs, either through an increased co-pay, co-insurance or deductible amount.
